you’ve seen it: pop-ups, banners, best-practice nudges, “hard stops,” drug interaction warnings, overdue screening
prompts, and the occasional alert that feels like it was written by a very anxious robot.
These reminders can absolutely improve carecatching medication mistakes, preventing missed vaccinations, and helping
busy humans remember the tenth thing they needed to do. But here’s the uncomfortable truth: a reminder can be
confident and still be wrong, or technically correct but clinically unhelpful for the person in front of you.
So… are EMR reminders always right? No. Are they always wrong? Also no. Like most powerful tools, they’re only as
good as the data, logic, timing, and human factors behind them.

warnings, or suggestions during clinical work. Some are quiet (“FYI: patient due for colon cancer screening”), and
some are loud (“STOP: potential life-threatening drug interaction”). They can be:
- Medication safety alerts (drug–drug interactions, allergies, dosing, kidney function flags)
- Preventive care prompts (vaccines, screenings, chronic disease monitoring)
- Order sets and pathways (standardized bundles for common conditions)
- Risk prediction alerts (early warning for sepsis, deterioration, readmission risk)
- Policy and documentation reminders (consent, controlled substances, follow-up tasks)
In theory, they deliver “the right information to the right person at the right time.” In practice, they sometimes
deliver the right information to the right person at the wrong timelike a fire alarm that goes off every time you
make toast.
